Admission Process
To facilitate the admission review process, we are requesting the following information be provided for our review. The hospital discharge planner or social worker may be able to assist you in obtaining this information if a loved one is currently in the hospital. Once the information is obtained, our referral review team will be able to begin the assessment process and respond to you with an admission decision in a timely manner.
The following information can be directly faxed to 814-267-6213:
- Patients Name
- Diagnosis, defining reason for hospitalization Social History
- Treatment Needs
- Medication Needs
- History and Physical
- Weight
- Last 5-7 days nursing notes
- Last Therapy notes, if applicable
- Insurance / Payer source information
In most cases an admission decision can be made from this information. If our Nursing Department has additional medical questions, we will call you directly so that all the questions can be answered in a single phone call.
